J-1 SEVIS Forms and Instructions

These forms and instructions will lead you through the procedures for getting your J-1 SEVIS documents and assuring compliance with the SEVIS system and regulations.

DS-2019 Processing Times:

1.Three to four weeks needed for newly coming J-1 scholar documents.
2. One to two weeks needed for continuing J student and scholar documents.

UMN has a mandatory insurance requirement for all J-1s and J-2s

Note: If J-1 or J-2s do not yet have UMN ID numbers, your department must assign them using the procedures outlined in section A.3. above. Use this form for:

  • Extensions
  • Correct the Record
  • Reinstatement of J-1 Status
  • Amend DS-2019 to correct or update information
